Cancellations of Appointments

If you have to cancel your appointment, please give us at least 24 hours. The physician reserves the right to charge for missed appointments.


Financial Policy

We ask that ALL patients READ AND SIGN our FINANCIAL POLICY and PATIENT INFORMATION FORM prior to seeing the doctor.

If your problem is MEDICALLY NECESSARY, we MAY ACCEPT INSURANCE ASSIGNMENT. Please understand:
In cases where we are preferred providers, we accept their allowed fee schedules IN CASES WHERE REIMBURSEMENT IS APPLICABLE. Patients covered by other insurance plans come under our customary fee schedule. In either case, you are responsible for deductibles and co payments.

Payment in FULL is due at the time services are rendered UNLESS you have a Payment Plan. We accept the following forms of payment: CASH, CHECK, ZELLE, YOUR BANK ATM CARD, VISA, MASTERCARD and DISCOVER/NOVUS.


Fees

We request that you pay for all professional fees and services at the time of your visit. If you have any questions about your bill or need to work out a financing program, please call our office.


Insurance

For medically necessary charges, you will be given a walk out receipt that itemizes the professional and diagnostic services. The diagnosis and all required information is on the form. To submit your claim to your insurance company, fill out your portion of the claim form and attach it to the walk-out receipt. Mail the completed claim form to the insurance company according to their instructions.

We wish to stress that any reimbursement you may receive from your insurance company is strictly between you and your insurance company. Insurance companies frequently set fees that are below our customary charges. You are obligated for the full amount.

In some cases, we may bill as a courtesy. Please give your insurance card to the receptionist and we will let you know if our practice has a relationship with your insurance carrier.


Medicare

We DO participate in Medicare and accept assignment on medically necessary and approved procedures. We will submit the claims to Medicare and have payment sent directly to our practice.

Unless you have a Medigap or secondary policy, you are responsible for the co-pay. Paying your annual deductible is your responsibility.

You will be asked to sign a Medicare Advance Notice for services that are deemed likely NOT to be allowed by Medicare. Any claims not approved will then become your responsibility.


Financing

We participate with Care Credit. On your approved credit, an amount will be made available to you in the form of a credit card. Payment comes directly from Care Credit to our practice. Then, you will receive statements from Care Credit.

Zero interest payment options are available for a fixed period. Interest options are available with competitive rates.


Prescriptions

Our approaches to treatment very rarely require prescription medications. Normally, over-the-counter (OTC) medications are appropriate and sufficient for post-treatment pain control.


Parking

At our Los Angeles office, the parking lot behind the building is available, but we don’t validate. Street parking is available on Wilshire Blvd. and the surrounding streets. PLEASE READ ALL POSTED SIGNS.

At our Walnut office, complimentary parking is available in the shopping center’s open-air parking lot.
